DEADLINES
Review the Graduate Studies Sessional Dates for information on Add/Drop periods, i.e. the last date for course changes without divisional approval.
ADDING A COURSE
It is the responsibility of the student to confirm the availability of seats in a particular course, and to obtain the instructor's permission and signature prior to enrolment.
Students must complete an Academic Change Form to report any changes to their program of study. This includes changes in study status as well as course changes such as adding or dropping courses.

When adding or dropping a course, please fill in the form as follows (fill in bold sections):
1) Student number
a) NetID for Queen's email
b) Academic Year
c) Name
2) Present Program of Study
a) Check Master's Program
b) Degree Program "MES"
c) Type: Enter "I" for Research or "II" for Course
d) Do not enter anything in Concentration or Level
3) Requested Program of Study -use this only if you are making a change
4) Present Study Status
a) Attendance - Check applicable term (Fall, Winter, Spring/Summer)
b) Locale - "On Campus" or "Off Campus"
5) Requested Study Status -as above if making changes
6) Course Changes
a) Fill in Course Code (Number) e.g. ENSC 801 or GPHY 882
b) Section if applicable
c) Check applicable term
d) Wgt = weight of the course - most are .5
e) Significance to Program: Enter "Primary," unless the course is being audited
d) Instructor's Approval: Obtain signature/initial from instructor if the course is outside our department
7) Print Form
8) Program Approval:Obtain signatures from your supervisor and the Department Head (Brian Cumming) or Graduate Coordinator (Heather Jamieson).
9) Photocopy the form before taking it to Karen Topping Rm 3132 Biosciences.
