Students must register annually with DSO to receive services and exam accommodations. This means that you have to return each academic year for a review of the previous year's accommodations, how you managed academically, and any changes that may need to be implemented. It is your chance to let us know how your previous year went and how we can best continue to meet your needs.
NB. All students must register with DSO before November 7 to be accommodated for December exams, and before March 7 for finals in April.
Submit your Pre-registration Form and Documentation to DSO
Complete form the Pre-registration form. Documentation should be mailed, faxed, or sent electronically to DSO at least 2 weeks prior to your appointment. Confirm that the office has received your documentation at the time of your appointment; otherwise you may need to re-schedule.
Book an Appointment with Disability Services
Schedule an appointment to meet with an advisor to discuss appropriate accommodation. The initial meeting for first-time registrants will take about 1 hr, at which time you will complete and sign any necessary paperwork.

Collect Forms for Your Individualized Accommodation
Based on the initial intake and interview, students will receive accommodations specific to their disability. Where appropriate, students may be referred to other service providers. Students receiving accommodations will be provided with:
- Accommodation Letter
- Template letter of self advocacy
Returning students must pre-register online. Your registration and accommodations expire every year, so make sure to renew your registration with DSO prior to November 7th and March 7th to receive your exam accommodations.
Collect Forms for Your Individualized Accommodation:
Once you have re-registered on line, you still have to go to the office to sign all forms and receive your accommodation letter. This does not require an appointment; check the DSO website for scheduled pick-up and drop-in times.
Why must I register with Disability Services every year?
Regular feedback allows students and staff to know what's working, and what needs to be changed or modified. By renewing your accommodations annually, they reflect your current accommodation needs as your move through your program of study.
