Forms

Forms is a new part of the Office 365 suite of software. Forms allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations and more. When you create a form, you can invite others to respond to it using any web browser, even on mobile devices. As results are submitted, you can use built-in analytics to evaluate responses. Form data, such as quiz results, can be easily exported to Excel for additional analysis.

Important Note

In accordance with the Queen's Policy on Administrative Student Surveys, "all administrative student surveys are expected to utilize Queen's survey software (currently Qualtrics) unless the researcher(s) and Institutional Research & Planning agree that special circumstances warrant use of another system."

In addition, users acknowledge and agree that they will not use Forms to conduct surveys that put Queen's in violation of FIPPA, PHIPA and/or PIPEDA, or collect other personal or confidential information without appropriate approval. Getting Started

Who Can Have It?

  • Forms is part of the Office 365 suite of cloud-based services available to all current students, staff and faculty of Queen's University with an active Queen's NetID@queensu.ca email account.
  • Alumni and retired staff and faculty are not eligible for Forms.

Are There Any Costs?

  • Forms is part of Office 365 service and provided at no cost to eligible users.
  • Users are responsible for any personal ISP/cellular arising data charges.

How Do I Get It?

What if I Leave Queen's?

  • Please see the Office 365 section of the Service Termination Dates page for details on when service will be discontinued following departure from Queen's.
