The Ontario Freedom of Information and Protection of Privacy Act now requires students to complete a Letter of Reference Consent Form before letters of reference can be written by faculty. This form can be faxed to the department at 613.533.6558. The original of this form must then be forwarded to the instructor.

If a student wishes to request an incomplete mark (IN), they must complete a Permission for Incomplete Grade and meet with the instructor of the class to have this request approved.
