Graduate - Forms

  • Academic Change Form [PDF] - to be completed by graduate student to add or drop or audit a graduate level course.  Must be signed by supervisor and submitted to the SKHS Graduate Assistant in KHS 206 for final approval. This document is essential in keeping your transcript up-to-date and accurate.  Failure to submit an A/C for each and every course add or drop will cause delay in preparing you to orally defend your thesis and convocate.
  • SKHS Graduate Seminar Series Exemption Form - to be completed on the Monday preceding the Seminar.  Must be signed by graduate student and supervisor and submitted to the SKHS Graduate Assistant in KHS 206 for recording.
  • Accessibility Education Online Tutorial - available through The Equity Office at Queen's University. This online tutorial is mandatory for all graduate students, faculty members and staff.  Graduate students will see the course preloaded to their transcript as AODA 802.
  • Advisory Committee / Thesis Proposal Meeting Form - to be completed during your thesis proposal or other advisory meeting(s).  Use one form for each meeting.  Copies will be provided to student, committee members, and a copy retained in your SKHS graduate student file.
