A NEW General Research Ethics Board (GREB) Standard Application Form.

As of Monday, October 17, 2016 the submission of the NEW GREB Standard Application Form in TRAQ should be used for all new GREB applications.

Please note: the current version of the application form should only be used to finalize approvals of applications that are currently in progress in TRAQ.
As of 11:59 p.m. on December 18, 2016, the current version of the GREB Application Form will be deactivated.
As of 12:00 a.m. on December 19, 2016, only the NEW GREB Standard Application Form will be active and accepted for standard GREB ethics submissions.
