Survey Inventory Form

Individuals and units undertaking administrative surveys are reminded to review the Administrative Student Survey Policy (ASSP). Under the AASP, the Office of Institutional Research and Planning is mandated to (among other things):

  • Maintain an inventory of surveys to inform all faculty and staff researchers of other survey activity and facilitate scheduling decisions.
  • Serve as the sole provider of student survey samples (and in most cases, faculty and staff samples) to allow us to avoid overlapping samples and survey fatigue.

This form is the means by which we maintain the survey inventory and determine scheduling and sample distribution for all student surveys. Please complete AT LEAST 15 days prior to the launch of your survey.

Completion of this Inventory is REQUIRED for:

  • Administrative surveys of students, alumni, faculty and staff (exceptions below)

Completion of this Inventory is RECOMMENDED for:

  • Faculty research projects

Completion of this Inventory is NOT REQUIRED for:

  • Surveys with a population of less than 200 persons
  • Event invitations
  • Responses related to the performance of an employee's responsibilities and of a student's academic requirements
  • Surveys for the purpose of course (course evaluations, feedback, etc.) or program improvement (curriculum or content surveys, quality assurance, etc.)
  • Completion of forms required to access or receive services
  • Responses/reactions to participation in group-targeted events (i.e. post-event surveys)
  • Surveys of external populations (i.e. entirely outside the Queen's community of applicants, students, alumni, faculty and staff)
