Forms FAQ

In accordance with the Queen's Policy on Administrative Student Surveys, "all administrative student surveys are expected to utilize Queen's survey software (currently Qualtrics) unless the researcher(s) and Institutional Research & Planning agree that special circumstances warrant use of another system."

In addition, users acknowledge and agree that they will not use Forms to conduct surveys that put Queen's in violation of FIPPA, PHIPA and/or PIPEDA, or collect other personal or confidential information without appropriate approval. Q. What is Forms?
Forms allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations and more. When you create a form, you can invite others to respond to it using any web browser, even on mobile devices. As results are submitted, you can use built-in analytics to evaluate responses. Form data, such as quiz results, can be easily exported to Excel for additional analysis.
Q. How do I access Forms on the web?
Forms can be accessed from the Office 365 app launcher after logging in to Office 365 at: or from your web browser by visiting and then signing in using your Queen's Office 365 account. Microsoft Forms is optimized for Internet Explorer 10+, Edge, Firefox (latest version), Chrome (latest version) and for Android (latest version) and iOS (latest version) web browsers.
Q. Can forms be completed by individuals outside of Queen's University?
Yes, you can set options to 'Anyone with a link (sign-in not required)' or 'Only people in my organization'. See Send your form to others for more information.
Q. Is there a Forms mobile app?
Forms are available through desktop and mobile browsers, and users can both create and complete Forms within the browser. This ensures that the widest number of people can reply to your Form without having to download and use a separate app.
Q. Can I embed Forms into a webpage?
Yes. Forms can be easily embedded into web pages via the embed code provided in the Send tab in the upper-right hand corner of the Forms page.
Q. Can I embed a form in Ofice Sway?

Q. Where geographically is the Form data stored?
Forms data is stored on servers located in the United States.
Q. What are some of the features and functionality that Forms supports?
Forms allows embedding images or videos, themes, question branching and co-authoring of forms using Office 365 Groups. Extensions through developer APIs are currently not available for Forms. Forms will evolve over time and additional functionality will be added based on customer feedback.
Q. Is there an option for respondents to see Forms results?
To create a summary page of your form or quiz that can be shared with respondents, follow these steps. In Microsoft Forms, open the form for which you want to share the results, and then click the Responses tab. Click the '...' located above the Open in Excel button and select Create a summary link from the drop down. You can now copy and share this link with others.
