Planning & Renovation Request Form

University Planning (UP) and Physical Plant Service (PPS) work closely together to develop and modify the University's physical environment in order to create and maintain a quality learning and work environment for all members of the Queen's community.  

This form may be used to request the following services:

  • Planning, including requests for new space and changes to space usage;
  • Renovations or alterations to existing space, or for estimates to plan such work; and
  • Design consultation and support to plan or reconfigure furniture, equipment and fixtures.

Project Approval

Please provide the name and contact information for the approval authority for this project.  When you submit your form, it will be automatically directed to the address in the Approver Email field below.  Please ensure that your form is submitted to the authorized approver for your Faculty or Shared Service.
