The health and safety of the Queen’s and Kingston communities remains our highest priority. All individuals who are participating in university activities in person must be vaccinated. Students and employees must complete the following vaccination status declaration form. Individuals who declare they are fully vaccinated will be required to upload proof of their vaccination status.
Any false information or misrepresentation on your vaccination status declaration form will be grounds for disciplinary action by the university.
The information collected through the vaccination status declaration form will be kept secure and confidential within the university. Personal information is collected under the authority of the Queen’s University Royal Charter, 1841, as amended, and will be used for the following purposes regarding COVID-19 vaccination status:
- collect and validate proof of vaccination for those who wish to access the university’s campus and facilities
- monitor compliance with university vaccination requirements protocols
- maximize safe operations on campus
- collect aggregate information on the overall immunity level of the campus population
- guide and frame operational decisions and outbreak management activities
The information you provide, and any information provided in any follow-up, may be shared with a limited number of designated university employees if required and only for the purpose of fulfilling their responsibilities relating to the health and safety of the university community.
You understand that it is your responsibility to inform yourself about the benefits and risks of COVID-19 vaccination in conjunction with your healthcare provider.
