COVID-19 Vaccination Status Declaration

The health and safety of the Queen’s and Kingston communities remains our highest priority. Though no longer mandatory to participate in university activities, students, faculty, staff, volunteers, and guests are encouraged to continue to follow best practices for controlling disease transmission, including getting vaccinated and boosted.

Note that some faculties—like the Faculty of Health Sciences—can have vaccination requirements which may be different than guidance provided here. Please consult your faculty directly for details.

Students and employees who must declare their vaccination status and upload proof of their vaccination status may complete the following vaccination status declaration form.

Any false information or misrepresentation on your vaccination status declaration form will be grounds for disciplinary action by the university.

The information collected through the vaccination status declaration form will be kept secure and confidential within the university. Personal information is collected under the authority of the Queen’s University Royal Charter, 1841, as amended, and will be used for the following purposes regarding COVID-19 vaccination status:

  • collect and validate proof of vaccination for those who wish to access the university’s campus and facilities
  • monitor compliance with university vaccination requirements protocols
  • maximize safe operations on campus
  • collect aggregate information on the overall immunity level of the campus population
  • guide and frame operational decisions and outbreak management activities

The information you provide, and any information provided in any follow-up, may be shared with a limited number of designated university employees if required and only for the purpose of fulfilling their responsibilities relating to the health and safety of the university community.

You understand that it is your responsibility to inform yourself about the benefits and risks of COVID-19 vaccination in conjunction with your healthcare provider.
