Students new to QSAS

To determine if you are eligible for online registration with QSAS, please answer the following questions:

  1. Do you know your Queen's NetID and password?
  2. Are you requesting an appointment with QSAS for the first time?
  3. Do you have documentation verifying your disability or health condition as described in our Documentation Requirements?
Follow these steps:
1. Gather your documentation

Review our Disability Documentation Requirements and obtain the recessary documentation from your health care provider. You are encouraged to utilize our Disability Documentation Form (DDF) .

If you have a learning disability , we will require you to provide a copy of your psychoeducational or neuropsychological assessment report that has been completed by a registered psychologist, neuropsychologist, or psychological associate within the last three (3) years or after age 18.

Students are encouraged to submit a copy of their most recent IEP and/or post-secondary accommodation plan as supporting documentation. However, this documentation is not sufficient on its own to provide academic accommodations at Queen’s.

2. Upload your disability documentation through our secure portal.

If you are unable to upload your documentation or experience any difficulties, please contact our Data Administrator

3. Our Intake Coordinator will contact you via your Queen’s email about next steps

If your documentation is sufficient, you will be invited to attend an intake appointment with one of our Accessibility Advisors.

If we require additional information, we will be in touch with you about what information is required to proceed with registration.
