Equity Reporting Forms (Appointments and RTPC)
The Equity Office is mandated to collect, track and report on equity data as it pertains to the faculty applicants. Under the Queens Univeristy Faculty Association (QUFA) collective agreement, the equity data is further used to inform the Employment Equity Representative (EE Rep) during the hiring process and the Equity Office in completing the annual compliance reports to the Joint Committee for the Administration of the Agreement (JCAA). The Faculty Appointments Database (FAD) has replaced the paper version of the Equity Reporting Forms. Access to the application can be found here: https://eservices.queensu.ca/apps/equityfacultysearch/. The EE Rep will need to begin the process by contacting the Equity Office at equity@queensu.ca for initial access to the application. The revised forms are available in hard copy by contacting the Equity Office. Please note that the forms are for use by Queen's University only. Permission is needed for reproducing these forms. If you would like these forms in another format, please contact the Equity Office at equity@queensu.ca
