1. I wish to anonymously share information about an incident of harassment, discrimination, or bias/hate.

Submitting an IN–SIGHT form, you understand:

  • That the university will be limited in its ability to respond to your specific circumstances;
  • That the information collected is shared in summary form (no identifying information) to help Queen's better understand the experiences of Queen's community members and to create more inclusive living, learning and working environments;
  • That your submission is NOT a formal Complaint or Report under the Harassment and Discrimination Prevention and Response Policy;
  • That you will NOT be contacted about your submission.

 

 

2. I wish to speak with an advisor to discuss an experience, learn about options, or connect to supports.

Submitting a Client Intake Form, you understand:

 

  • That you will confidentially share your contact information with the Human Rights Advisory Services (HRAS) to arrange an appointment with an advisor to discuss your situation;
  • That consultations with the HRAS are confidential, except in circumstances where information must be shared to address a risk to the health and safety of members of the University Community (e.g., where there are reasonable grounds to believe that an individual is at risk of self harm; there are reasonable grounds to believe that an individual may pose a threat or risk to members of the University Community.)
  • That client intake form submissions are received by Human Rights Advisory Services Monday to Friday, 8:30am – 4:30pm and that you will be contacted to arrange an appointment during these regular office hours.
