Price List for Uninsured Services

Some services are not covered by primary or supplementary health insurance. In addition, if students do not currently have insurance coverage, they can still receive care but will have to pay a fee.

Prices may change therefore please confirm rates with reception prior to appointments. Email wellness.services@queensu.ca regarding billing, invoicing or SOLUS account charges.

All uninsured appointments with a physician will be billed in accordance with the current Ontario Medical Association (OMA) rates: 

  • Minor Assessment – $59.40
  • Intermediate Assessment - $77.20
  • Psychotherapy per ½ hour - $143.70
  • Mental Health Care ½ hour - $143.70

  • Completing health forms for academic programs (e.g., immunization review) – $20
  • Longer nursing appointments booked for other reasons – $10 per 10 minutes 

Tuberculosis (TB) Testing

There is no charge for Queen's mandated programs (e.g., medicine and nursing) however there is a fee for non-mandated TB testing (i.e., for volunteering, travel, etc.)

  • 1-step TB test $30
  • 2-step TB test $60

TB testing checks to see if students have been infected with tuberculosis. This includes 4 separate appointments spread over several weeks which is why they can only be booked in-person or by phone.

If students have documentation of a prior 2-step TB test, they can book a 1-step TB test which will include 2 appointments 48-72 hours apart.

  • All lesions treated non-surgically for cosmetic purposes, including warts – $15
    • Genital and plantar wart treatments are covered by Provincial Health Insurance Plans and University Health Insurance Plans       
  • Excision of single lesion-cosmetic – $85
  • Uninsured injections – $10

To have your records sent to another physician/location or to obtain copies of your medical records please complete the Consent Regarding Personal Health Information form indicating specifically what you require (e.g., immunization record, entire chart, recent bloodwork) and drop it off at Student Wellness Services or send it by email to: sws.records@queensu.ca

Students can also obtain records in person without filling out a release of information by showing identification that has a picture at the our reception desk in Mitchell Hall.

  • Transferring records - $40
  • Preparation of OSAP/QSAS/Other forms - $40
  • Photocopying – for transfer of larger files or in situations where a record must be reviewed by a physician or consultant, the listed hourly rate and photocopying fees will apply – $0.50 per page
  • Chart summary family physician – $200/hr
  • Chart summary psychiatrist – $400/hr
  • Certificate of health (including examination) – $120
  • Driver’s license medical & form – $120
  • Preparation of miscellaneous medical reports based on hourly rate e.g. medico-legal reports:    
    • Family Physician $200/hr
    • Psychiatrist $400 /hr
  • Pre-employment assessment and certification – $120
  • Notes for work if deemed appropriate. Non-medical appointments with physicians for notes, etc. are uninsured by Provincial Health Insurance Plans and University Health Insurance Plans and will be billed at the Ontario Medical Association rates (see rates above) – $20
  • Third party physicals – $120
