All students registering with Queen's Student Accessibility Services (QSAS) must complete the student intake form located in Ventus and submit documentation that meets the documentation criteria. Students must provide documentation that:
- Confirms that they are a student with a disability
- Confirmation of disability from a health care professional is required, however, disclosure of the diagnosis is not required.
- Contains information about functional limitations within the academic environment.
- Indicates the duration and severity of the disability or impairment.
Examples of documentation:
- Psychoeducational Assessment (completed in the last five (5) years or after the age of 18.
- OSAP Disability Verification Form.
- QSAS Verification Form.
- Other medical documentation that includes documentation criteria listed above.
Students who submit documentation meeting the QSAS Documentation Criteria may receive permanent accommodations.
Students who are unable to submit documentation meeting the QSAS Documentation Criteria, but who are able to submit some documentation, may receive interim accommodations.
QSAS Advisors may request additional documentation.
Verification Forms
Examples include:
- Attention-Deficit Hyperactivity Disorder (ADHD), Attention Deficit Disorder (ADD)
- Learning disabilities including, but not limited to, dyslexia, dysgraphia, auditory processing disorders, etc.
- Neurodiverse perspectives including, but not limited to, Autism Spectrum Disorder (ASD), Tourette Syndrome, etc.
QSAS Documentation Criteria (one of the following):
- QSAS Verification Form for Attention/Learning/Neurodiversity
- Psychoeducational Assessment – completed within the last 5 years or after the age of 18
- Other medical documentation from a professional health care provider that meets the QSAS documentation criteria
When enrolled in courses students can upload their medical documentation to Queen's Student Accessibility Services (QSAS) through the Ventus Student Portal.

Examples include:
- Medical conditions including, but not limited to, diabetes, heart condition, cancer, Crohn’s disease, irritable bowel syndrome, etc.
- Brain injuries including, but not limited to, concussion, acquired brain injury (ABI), etc.
QSAS Documentation Criteria (one of the following):
- QSAS Verification Form for Medical/Brain Injury
- Neurological assessment – completed in the last 3 years
- Other medical documentation from a professional health care provider that meets the QSAS documentation criteria
Examples include:
- Major depressive disorder; generalized anxiety disorder; borderline personality disorder; eating disorder; etc.
QSAS Documentation Criteria (one of the following):
- QSAS Verification Form for Mental Health
- Neurological assessment – completed in the last 1 year
- Psychoeducational assessment – completed in the last 5 years
- Psychological assessment - completed in the last 1 year
- Psychiatric assessment/report – completed in the last 1 year
- Other medical documentation from a professional health care provider that meets the QSAS documentation criteria
Examples include:
- Mobility impairments related to, but not limited to, cerebral palsy; developmental coordination disorder; etc.
- Hearing loss related to, but not limited to, Deaf, deafened, hard of hearing (may be used to rule out hearing issues for individuals experiencing Central Auditory Processing Disorder)
- Vision loss related to, but not limited to, macular degeneration; cataracts; glaucoma; diabetic retinopathy; blindness
QSAS Documentation Criteria (one of the following):
- QSAS Verification Form for Physical/Hearing/Vision
- Audiogram – Completed in the last 1 year
- Audiology Assessment
- Vision Assessment
- Canadian National Institute for the Blind (CNIB) Card
Submitting Documentation

Other Documentation
While we will eventually require documentation that meets the QSAS documentation criteria for permanent accommodation, we can begin to support all students experiencing disability-related impairments, or suspected disability related impairments that are impacting their academics.
